IPLEDGE is the pregnancy gate. This glint is the blood gate. Pretreatment fasting lipids, then repeats until the pattern is clear - usually by week four.
High-risk groups (diabetes, obesity, heavy alcohol, known lipid disorder, family lipid disease) need more frequent checks if 10 mg proceeds. Uncontrolled hypertriglyceridemia is a reason to stop, not to 'stay on 10 mg because it is small'.

Daily paracetamol for a new headache on 10 mg is both an LFT confounder and a reason to ask whether the headache is ordinary dryness or raised pressure.

Liver enzymes are the other column
LFTs are checked before and during. Hepatitis and enzyme elevations are labelled. A quiet 10 mg does not make a rising ALT a mystery vitamin.
Paracetamol habit, herbal 'skin teas', and viral illness all move LFTs. Bring the list.

The 36-hour gap is a lab instruction
After drinking, wait at least 36 hours before fasting lipids. A Thursday party and a Friday 08:00 draw is how people look like they failed 10 mg when they failed the fast.
Alcohol also raises triglycerides on its own. 'I only drink on 10 mg weekends' is still a lipid story.
Do not skip the 10 mg the night before labs unless told. The draw is about the drug-plus-you pattern, not a drug holiday.
The night-before script for a 10 mg fasting draw
Last alcohol at least 36 hours before the tube. Last food as they define fasting, usually from midnight. Water yes. Bacon roll no.
Take 10 mg as usual unless they told you to hold. The draw is the pattern, not a drug holiday.
Write last drink time and last food time on a slip in the bag with the form.
If you broke the fast, rebook. A fed lipid on 10 mg is how people get a fake crisis or a fake all-clear.
Night-shift: pick a morning you can actually wake and fast, not the morning after a canteen fry-up.

If diarrhoea from a virus wrecked the week, say so. Illness moves lipids and LFTs.
Book the next draw before you leave phlebotomy if they want a repeat inside the 30-day window.
Keep the PDF. The next locum will ask for a baseline you already paid for.
What the 25 / 15 / 7 percents mean at 10 mg
Trial language: marked triglyceride elevations in about one in four, HDL down in about 15 percent, cholesterol up in about 7 percent. Effects were reversible after stop in those trials. Some people lowered triglycerides by cutting fat, alcohol, and weight, or by reducing dose, while staying on drug.
Triglycerides above 800 mg/dL have been reported. Pancreatitis risk sits on that extreme, not on a mild bump. Still, a rising trend on 10 mg is a clinic fax, not a wait-until-month-four plan.
The 10 mg start may move lipids less than 1 mg/kg - or not. Do not skip the baseline because you expect a quiet capsule.

Herbal 'liver cleanse' teas are a second variable. Stop them before you blame 10 mg for an enzyme bump.
Viral illness moves LFTs. A week-four draw during flu is a messy 10 mg story.
Alcohol is both a 36-hour lipid problem and an LFT problem. Two reasons to date the last drink.
Do not add ibuprofen stacks for back pain from the gym without naming them. The 10 mg is not the only liver visitor.
When 10 mg should pause for a number
Uncontrolled triglycerides, symptomatic pancreatitis picture, significant hepatitis, or a pregnancy-programme failure: stop and call. Dose reduction is a specialist tool, not a home snap of the 10 mg.
A mildly high triglyceride at week two with a plan (diet, alcohol cut, early repeat) may continue 10 mg. That plan is written.
Do not add a statin from a relative's box to 'protect' a 10 mg course.
How to book the week-four 10 mg labs
Book the bloods before the 30-day supply ends so the next window has numbers. Evening shifts: pick a morning you can fast.
If the 10 mg is split BID, still fast for the lipid tube. The clock of the capsule is not the clock of the draw.
Keep copies. Locums restart 10 mg without a baseline when the first hospital PDF is lost.

Mmol/L versus mg/dL on a 10 mg printout
UK labs often report triglycerides in mmol/L. US labels talk in mg/dL. The 800 mg/dL crisis number is about 9 mmol/L. Do not panic at 2.5 mmol/L as if it were 800.
Do not be casual at 4 mmol/L and rising on 10 mg either. Call with the units they use.
A conversion error is how people stop 10 mg for a breakfast and how people ignore a real spike.
Keep the same lab if you can. Switching hospitals mid-course adds method noise.
Write the unit on the fridge next to the 36-hour alcohol rule.

Know your units. 2.8 mmol/L is not 800 mg/dL. Rising from your baseline still goes to the specialist. Uncontrolled highs stop 10 mg.

A new statin or a new oral contraceptive can move lipids and LFTs. Name both at the draw. Do not add a supplement 'for the liver' the week of the tube.
Night-shift fasting is still eight hours without calories. Black coffee only if the lab allows it. A 04:00 kebab then a 09:00 lipid is a wasted 10 mg month.
Pregnancy tests and lipids share a calendar, not a fridge. A lipid-only visit does not reopen a closed iPLEDGE window. Both must be current before the next 10 mg box.
A GP who repeats 'your LFTs are fine' without the number still leaves you without a baseline. Ask for the printout. Week-eight 10 mg decisions need last month's digits, not a vibe.
