Why George Street locks Levitra at 2.5 mg
Open this frame when the bag says vardenafil 2.5 mg, or when a strong 3A4 inhibitor makes 2.5 mg the only legal film.
The 2.5 mg film exists because some charts cannot take the usual 10 mg start. Ritonavir: no more than 2.5 mg in a 72-hour period. Cobicistat: the same 72-hour cap. Indinavir, saquinavir, atazanavir, ketoconazole 400 mg daily, itraconazole 400 mg daily, and clarithromycin: no more than 2.5 mg in 24 hours.
That is a ceiling, not a lifestyle pick. For most men the labelled start is still 10 mg, taken orally as needed, about 60 minutes before sexual activity (UK wording: 25 to 60 minutes). The dose may rise to 20 mg or fall to 5 mg. Maximum frequency is once per day. Sexual stimulation is required.
A Dosage headline that treats 2.5 mg as the default first swallow for a healthy 40-year-old has misread the tablet strengths. A headline that skips 2.5 mg entirely has misread the CYP table. This frame prints the small film because that is the lock, and it prints the usual 10 mg so nobody leaves thinking 2.5 mg is 'the weak one that never works'.

Orally disintegrating vardenafil is a different product
Staxyn and other ODT vardenafil products are not interchangeable with a 2.5 mg film-coated Levitra tablet on a milligram-for-milligram hunch. Bioavailability and food rules differ. If the chemist substituted an ODT, ask which product landed in the bag before you compare this frame's 2.5 mg cash row to what you swallowed.
Some generic vardenafil tablets contain aspartame. Each 2.5 mg tablet may carry about 0.7 mg phenylalanine. That matters in phenylketonuria. It does not change the CYP cap.

George Street reprints a sourced GoodRx national row for generic vardenafil 2.5 mg x 30 - the small-start lock - and does not fill the bottle. Twenty-milligram ten-counts are a different board. QT first. GoodRx stays in the caption.
NHS stock gaps sometimes push men from Levitra-brand thinking toward sildenafil or tadalafil. That is formulary logistics, not proof that 2.5 mg failed. Ask the chemist which INN is actually available before ordering strips from abroad.

Most clean charts still start at 10 mg
If there is no strong CYP3A4 inhibitor, no cobicistat, no moderate hepatic impairment, and no stacked alpha-blocker problem, 10 mg is the recommended start. Twenty milligrams is the ceiling. Five milligrams is the usual step down. Two-point-five is the film below that, reserved for the cuts in the next sections.
Elderly men (>=65) have higher exposure: mean Cmax about 34% higher and AUC about 52% higher than younger men. A 5 mg start should be considered. That is not the 2.5 mg lock. It is the geriatric consideration on a clean CYP chart.
Do not swallow a 20 mg film because a mate called 10 mg slow. Peak timing is not the same as peak height. The onset layer exists for that confusion. A second tablet the same day is already a label breach.
A 2.5 mg foil in a man with a clean chart is often a stock or transcription mix, not a lifestyle choice. Ask the chemist which strength was intended before you decide the INN 'never works'. The lock on this frame is the small film when the CYP table demands it, not a campaign to start everyone at 2.5 mg.

A high-fat meal can cut Cmax by 18 to 50 percent
Vardenafil is rapidly absorbed. Absolute bioavailability is about 15%. After a single 20 mg dose in healthy volunteers, Tmax usually sits between 30 minutes and 2 hours, median 60 minutes, fasting. Two food-effect studies showed high-fat meals reduced Cmax by 18% to 50%.
The UK SmPC says Levitra can be taken with or without food, but onset may be delayed with a high-fat meal. That is closer to sildenafil than to tadalafil. A steak-and-chips evening is a timing problem, not proof that 2.5 mg 'never works'.
Total body clearance is about 56 L/h. Terminal half-life of parent and the M1 metabolite is about 4-5 hours on a clean chart. Excretion is mostly as metabolites in faeces (about 91-95%), with 2-6% in urine. On ritonavir the half-life story changes; see the CYP section rather than this paragraph.

The 10 mg and 80 mg QT study still governs 2.5 mg
A thorough QT study in 59 healthy men aged 45-60 measured the interval at one hour, near average peak concentration. Therapeutic 10 mg and supratherapeutic 80 mg (four times the highest recommended dose) both prolonged QTc versus placebo, with moxifloxacin as the active control. Heart rate rose about 5 beats/minute at 10 mg and about 6 at 80 mg.
The 80 mg dose was chosen because it covers concentrations seen when a low vardenafil dose (5 mg) sits with ritonavir 600 mg twice daily. That is the same interaction that forces the 2.5 mg / 72 hour cap. QT talk and the small film are one story, not two rumours.
Men with congenital long QT, or men on class IA or class III anti-arrhythmics, should avoid vardenafil. That is a labelled avoid, not a 'monitor and see' hedge. A normal ECG last year does not clear a new amiodarone start this month. Sudden syncope with palpitations after a dose is 999, not a glint-thread delay.
Cardiac stops that outrank a fast-peak rumour
- Congenital long QT or class IA/III anti-arrhythmics: avoid vardenafil
- Ritonavir / cobicistat: 2.5 mg in 72 hours, and the QT study used that concentration band
- Clarithromycin: 2.5 mg / 24 h and a QT conversation
- Nitrates and riociguat: still contraindicated at 2.5 mg
A stable alpha-blocker starts at 5 mg, or 2.5 with CYP
If the man is already stable on an alpha-blocker, PDE5 inhibitors start at the lowest recommended dose. For vardenafil that is 5 mg. If a listed CYP3A4 inhibitor is also on the chart, the start becomes 2.5 mg. Do not start both drugs the same week as a home experiment.
Standing dizziness after the first combined evening is a sit-down and call problem, not a reason to add a second 2.5 mg film. Stepwise rises in the alpha-blocker can drop pressure further while vardenafil is on board.
Nitrates remain a hard contraindication at every vardenafil strength, including 2.5 mg. There is no 'tiny film' exception for GTN, isosorbide, or poppers.
A man who started tamsulosin last Tuesday and wants Levitra on Friday is not 'stable on alpha-blocker therapy'. Stable means weeks, not a shared shopping bag. Sit if the first combined evening goes grey. Do not add a second 2.5 mg film to chase the evening.
Age 65 and Child-Pugh B do not default to 2.5 mg
Phase 3 trials included more than 834 elderly men and did not show a different safety story at 5, 10, or 20 mg versus younger men. Exposure is still higher, so a 5 mg start should be considered at >=65. That is one step above the 2.5 mg lock, not the same sentence.
Moderate hepatic impairment (Child-Pugh B): start 5 mg, maximum 10 mg. After a 10 mg dose in that group, Cmax rose about 130% and AUC about 160% versus healthy controls. Severe hepatic impairment is outside casual ED prescribing on this frame.
Severe renal impairment (CrCl under 30 mL/min): a 5 mg start should be considered. Mild-to-moderate renal impairment does not require an automatic cut. None of those sentences replace the CYP table when ritonavir is already on the chart.

Levitra's smallest film is 2.5 mg. On a clean chart the start is still 10 mg. On ritonavir or cobicistat the cap is 2.5 mg in 72 hours. High-fat meals cut the peak. Class IA/III anti-arrhythmics and congenital long QT stay off this INN. Nitrates stay banned at every strength.
